Please follow the directions below:

Sign and complete the top portion
Take paperwork to the registrar at your college or university
The registrar must complete the certification of full-time attendance in addition, you or the registrar must return it to the address on top of the form
After the form is received, it will be sent to the Treasurer for writing of the check. Please be advised the overall process can take approximately two weeks from the time that we receive the completed form.
