Immunization Records

Due to the Coronavirus Covid-19 outbreak, Student Records is temporarily limited in its ability to process record requests.

Immunization records may be obtained from the Student Records Department if the student's birthdate was on or after January 1st, 1970 and immunization records were presented when student enrolled. The Student Records Department requires the following in order to release your immunization records to you:

  • a valid photo ID such as a license or passport
  • a phone number where you can be reached
  • the name that you were registered under when you were a student
  • your birthdate
  • the name of the school that you last attended in our district
  • your actual signature-not electronic signature

Your student ID number is helpful in finding your record, but not required. You can fax this information to 909.473.0052. At this time, all requests will be processed to an email address only.
